Request for Change of Name in the Register

 

Procedure for Requesting Change of Name:

  • A member of the College may request a change of his or her name in the register, provided the change of name request complies with College by-laws and is approved by the Registrar.
  • To request a change of name, please complete the required form and return it to the College with required supporting documents and payment of fee. Ensure that you follow all instructions in the form:

Name in the Register

  • The full legal name of the member must be entered in the register.   Abbreviations, nicknames, alternate names or initials are not permitted.
  • Rules for entry of name in the register and change of name are set out in the College’s General by-law (s. 48):
    • 48. (1) A member's name in the register shall be the member's full name and consistent with the name used by the member in his or her undergraduate medical training, as supported by documentary evidence.
    • (2) The registrar may direct that a member's name, which is not the name used by the member in his or her medical training, be entered in the register if the member satisfies the registrar that the member has validly changed his or her name since undergraduate medical training and that the use of the newer name is not for an improper purpose.
    • (3) The registrar may give a direction under subsection (2) before or after the initial entry of the member's name in the register.

Public Information (General By-Law, s. 48, s. 49(1)1 and s. 50.1(1))

  • Every member’s full name must be entered in the register and be publicly available in the College’s website.  Abbreviations, nicknames, alternate names or initials are not permitted.
  • Every subsequent change in the member’s name that is approved must be entered in the register and be publicly available.  Former names entered in the register will continue to be publicly available.

Professional Misconduct

  • It is professional misconduct for a member to use a name other than his or her name as set out in the register in the course of providing or offering to provide services within the scope of practice of the profession. (s. 1 (1) 15, O. Reg. 856/93)
