How do I obtain a replacement certificate of registration?
The certificate of registration is the official College document (size 8” x 11”) mailed to you upon registration that shows the class, date, and terms, conditions and limitations of your College registration.
To request a replacement certificate of registration, please contact Membership Services Department at the address below. Be sure to quote your registration number and explain briefly why you need a replacement.
The fee for replacement of a certificate of registration is $10 and can be obtained by completing the credit card authorization form.
The certificate will be mailed to your official mailing address, usually within two weeks from the date we receive your request and payment.
How do I obtain request a replacement membership certificate?
A membership certificate is the wallet-sized card certifying your current membership status.
After each annual renewal of your registration, your new membership certificate is available online from the College’s website. You can access and print it anytime in the current membership year by logging into your account in ‘CPSO Online.’
Please note that if you are a first-year member, the online membership certificate will not be available to you until you have completed your first online annual membership renewal. In the meantime, if you require a replacement, please contact Membership Services Department at the address below.
How do I request a duplicate receipt?
Access to renewal fee receipts for the years 2014 and onward are available online by logging into your account at ‘CPSO Online.’ Note: you must be an active member to access online receipts.
Receipts for application fees and other College services are not yet available online; they are mailed within 10 days of the date the payment is processed. Duplicate receipts for fees paid in the current year are issued at no charge. However, a $10 fee is required for duplicate receipt requests for payments made in prior years.
Duplicate receipt requests for the current year can be obtained by emailing email@example.com. Duplicate receipt requests for prior years can be obtained by completing the credit card authorization form.
