             As from September 24, 2014, the following is imposed as a term, condition and
            limitation on the certificate of registration held by Dr. Shui Hing Lee, in
            accordance with an undertaking and consent given by Dr. Lee to the College of
            Physicians and Surgeons of Ontario: 

                  Dr. Shui Hing Lee shall:
                  
                  (i)   advise all patients that I am not authorized to perform cataract
                        extraction and intraocular lens implant surgery independently; and
                  
                  (ii)  only perform cataract extraction and intraocular lens implant
                        surgery under the direct supervision of a supervising
                        ophthalmologist, acceptable to and approved by the College who must
                        be present throughout the entire surgical procedure and intervene
                        if necessary.
