            As from May 10, 2017, by order of the Quality Assurance Committee of the
            College of Physicians and Surgeons of Ontario, the following terms, conditions
            and limitations are imposed on the certificate of registration held by Dr. Alan
            Taniguchi:

            1.    Effective immediately, Dr. Taniguchi must obtain, at his own expense, a
                  clinical supervisor ("Clinical Supervisor") who is acceptable to the
                  College and who has signed an Undertaking in the form attached hereto as
                  Schedule "A".  The supervision will include the following:  

                  i.    Commencing on the date the Clinical Supervisor signs  Schedule "A",
                        Dr. Taniguchi must meet with the Clinical Supervisor every month
                        and review ten(10) of his long term care patient charts to discuss
                        Dr. Taniguchi's documentation, preventative care and maintenance
                        and his professionalism. 
                  
                  ii.   The Clinical Supervisor will report to the College every three
                        months.
                  
            2.    If Dr. Taniguchi is unable to obtain a Clinical Supervisor as set out in
                  paragraph 1 above, within fourteen days of the date of this Order, Dr.
                  Taniguchi must cease practising long term care immediately until such
                  time as he has obtained a Clinical Supervisor acceptable to the College. 

            3.    If Dr. Taniguchi is required to cease practicing long term care as a
                  result of section (2) above, this will constitute a term, condition or
                  limitation on his certificate of registration and said term, condition or
                  limitation will be included on the public register.

            4.    This Order will remain in effect until 11:59 p.m. on May 9, 2018, unless
                  rescinded or varied by the Quality Assurance Committee.

Specified Continuing Education and Remediation Program:

Where the Quality Assurance Committee has made a decision on or after June 1, 2016 in which the disposition includes a Specified Continuing Education and Remediation Program (“SCERP”), the College By-laws require that the elements of the SCERP be posted on the register. A SCERP is one of the dispositions that the Quality Assurance Committee may make in connection with a matter before it, and this disposition requires the member to complete an education and remediation program specified for the member.
