Registration Applications and Forms

For issuance of a certificate of registration, physicians must complete the applicable application form and all other credentialing and fee requirements set out in the instruction guide.

Update: As of October 2018, the College is accepting online applications for medical registration securely and easily through the national portal, physiciansapply.ca.

The online application is currently available as an option for all first time applicants to Ontario who meet the standards and qualifications for a certificate authorizing independent practice as outlined in the Ontario Regulation 865/93: Registration.  For more information about this exciting initiative, please review this guide.


  • Postgraduate Education Applications - Residents and Clinical Fellows

    Postgraduate Education Application — Canadian or US Graduates
    Application for graduates of accredited medical schools in Canada or the USA with a confirmed appointment to residency or clinical fellowship at one of six medical schools in Ontario.

    Postgraduate Education Application — Graduates outside Canada and the US - Clinical Fellows
    Application for international medical graduates with a confirmed appointment to clinical fellowship at one of six medical schools in Ontario.

    Postgraduate Education Application — Graduates outside Canada and the US - Residents
    Application for international medical graduates with a confirmed appointment to residency at one of six medical schools in Ontario.

    Candidates who already have a registration file in Ontario, i.e. returning clinical fellows, may contact the Applications and Credentials Department at 416-967-2617 to obtain a customized application package.

  • Postgraduate Education Applications - Electives

    Postgraduate Education Elective Application — Canadian or US Graduates
    Application for Canadian or US medical school graduates currently enrolled in a postgraduate medical education program at an accredited medical school in Canada or the US with a confirmed appointment to an elective (not to exceed 15 weeks) at one of six medical schools in Ontario.

    Postgraduate Education Elective Application — Graduates outside Canada or the US
    Application for international medical graduates currently enrolled in a postgraduate medical education program at an accredited medical school in Canada or the US with a confirmed appointment to an elective (not to exceed 15 weeks) at one of six medical schools in Ontario.

    IMG trainees currently enrolled in a postgraduate medical education program in the United States are eligible only if they passed the Medical Council of Canada Evaluating Examination (MCCEE) and have satisfactorily completed at least one year of the program.

    International Resident Electives Applicants currently enrolled in a residency program outside Canada or the US with a confirmed international resident elective appointment at an Ontario medical school must call 416-967-2617 to confirm eligibility and obtain a customized application package.

    Candidates who already have a registration file in Ontario, i.e. returning elective trainees, may contact the Applications and Credentials Department at 416-967-2617 to obtain a customized application package.

  • Applications for Certificate of Registration Authorizing Medical Practice

    Independent Practice – Canadian, US and International Medical Graduates
    This is a new online application for medical registration in Ontario available via physiciansapply.ca. This option is currently only available to first time applicants to Ontario. More information about this new initiative is available in our guide.

    If you wish to access to the online application, you must first submit the review of qualifications form and a copy of your curriculum vitae (i.e. resume) to inquiries@cpso.on.ca. Completion of this form is used to determine your eligibility for registration under the Ontario Regulation. Our review of your qualifications does not guarantee that you will be granted access to the online application, or awarded a Certificate of Registration to practice medicine in Ontario.

    Independent Practice — Canadian or US Graduates
    This application package is for Canadian or US medical school graduates who already have a registration file based on current or previous licensure in Ontario.

    Independent Practice — International Medical Graduates
    This application package is for international medical school graduates who already have a registration file based on current or previous licensure in Ontario.

    Canadian Free Trade Agreement
    If you wish to receive an application package on the basis of your out-of-province licence under the Canadian Free Trade Agreement (formally the Agreement on Internal Trade), you must submit the review of qualifications form, a copy of your current out of province license, and curriculum vitae (i.e. résumé) to inquiries@cpso.on.ca. Completion of this form is used to determine your eligibility for registration under the provisions in the Regulated Health Professions Act relating to the Canadian Free Trade Agreement, and the College’s registration regulations and policies authorizing medical practice in Ontario. Our review of your qualifications does not guarantee that you will be awarded a Certificate of Registration to practice medicine in Ontario.

  • Registration Policy Applications

    If you wish to receive an apply under any registration of the Council approved registration policies ( e.g. Restricted Certificates of Registration for Exam Eligible Candidates, Pathways 1, 2, 3 and 4, or Recognition of Certification without Examination by CFPC, etc.), you must submit the review of qualifications form and a copy of your curriculum vitae (i.e. resume) to inquiries@cpso.on.ca. Completion of this form is used to determine your eligibility for registration under the College’s registration regulations and policies authorizing medical practice in Ontario. Our review of your qualifications does not guarantee that you will be awarded a Certificate of Registration to practice medicine in Ontario.

  • Applications for Changing Scope of Practice and/or Re-entering Practice

    All physicians who wish to change their scope of practice and/or re-enter practice must participate in a College review process to demonstrate their competence in the area in which they intend to practise. The process for re-entry and change in scope of practice is individualized for each physician but, in general, includes a needs assessment, training, supervision, and a final assessment.   • Individual Credentialing Forms

Other classes of registration

Applications for other classes of registration (i.e. Academic Practice and Supervised Short Duration) may be obtained by contacting the Applications and Credentials Department. The standard registration requirements for each class of license in Ontario is available for review.

Candidates who already have a registration file in Ontario (i.e. current members or returning previous members) may contact the Applications and Credentials Department to obtain a customized application package.

College Registration Fees
When applying for a certificate of registration, applicants are required to pay both an Application Fee and Membership Fee as part of the regular requirements for registration.

Expedited Assessment
The College offers a service fee that allows for the expedited review of applications. The initial assessment for applicants who select this option will be less than three weeks, which is currently the service standard. Expedited review does not include the time required to issue a certificate. Expedited review is not available for applications with past complaints, investigations, or for applications that must be reviewed by the Registration Committee.

Review of Qualifications
Completion of this form is used to determine your eligibility for registration under the CPSOs registration regulations and policies authorizing medical practice in Ontario.

Blood Bourne Viruses (BBV) Policy
This policy requires physicians who intend to perform or assist in performing exposure prone procedures in Ontario (including new registrants) to be tested for HCV, HIV and HBV — if they have not been confirmed immune to HBV — before they commence these procedures. Providing a declaration of testing is part of new registrants’ initial applications to the CPSO.
