            As from February 13, 2017, the following terms, conditions and limitations are
            imposed on the certificate of registration held by Dr. Aras Jonas Balsys, in
            accordance with an undertaking and consent Dr. Balsys has given to the College
            of Physicians and Surgeons of Ontario:

                  Dr. Balsys has agreed to restrict his office-based practice at all of his
                  Practice Locations, including any Walk-In Clinic where he may work, to
                  routine, non-urgent, non-emergent consultations within his scope of
                  practice as a general internist, which includes respirology. If a patient
                  requires a procedure outside of his scope of practice, he shall make the
                  appropriate referral.
                  Dr. Balsys has agreed to restrict his practice to patients who have been
                  referred to him for consultation by another physician or nurse
                  practitioner who has assessed the patient prior to requesting the
                  consult, and to follow-up as necessary. Dr. Balsys may not attend on
                  patients without such referral.
                  Dr. Balsys does not have privileges at any hospital in Ontario or in any
                  other jurisdiction, with the exception of courtesy privileges at Humber
                  River Regional Hospital limited to pulmonary function testing. Dr. Balsys
                  has agreed not to seek hospital privileges in Ontario or in any other

Specified Continuing Education and Remediation Program:

A summary of a decision of the Inquiries, Complaints and Reports Committee in which the disposition includes a Specified Continuing Education and Remediation Program (“SCERP”) is required by the College By-laws to be posted on the register, along with a note if the decision has been appealed. A SCERP is one of the dispositions that the College’s Inquiries, Complaints and Reports Committee may make in connection with a matter before it, and this disposition requires the member to complete an education and remediation program specified for the member. A note will also be posted when all the elements of the SCERP have been completed. The summary will be removed from the register if the decision is overturned on appeal or review. Note that this requirement only applies to decisions arising out of a complaint dated on or after January 1, 2015 or if there was no complaint, the first appointment of investigators dated on or after January 1, 2015.
