Step 1: Information Sharing Consent 

While your name has been submitted by your hospital organization to enroll you in the CPSO QI Partnership for Hospitals program, by completing the information sharing consent, this confirms your participation in the program and your preference regarding information sharing about your CPSO Partnership program completion with your organization. 

Step 2: QI Activities Selection[view]

You will need to independently complete three activities from the list of included QI activities; the choice of activities is entirely up to you. You are welcome to complete additional activities to obtain CPD credits, to provide feedback on the menu of options within the program, and to inspire further QI ideas for your practice. 

Within this step, you are also asked to provide a brief reflection on your engagement in QI over the previous 5-year period. 

Step 3: QI Initiative Reflection Activity 

This short 3-question activity asks you to reflect on your understanding of the current QI initiative that you are participating in with your hospital peers. 

It also serves as a helpful record of the details of your participation in the QI Partnership program – as many hospital-based physicians are engaged in more than one QI initiative with more than one hospital organization. 

Step 4: QI Coaching  

After completing Steps 1-3, you will meet with a QI Coach to discuss your reflections from completing the activities and your participation in your organization’s QI initiative. Subsequently, the coach will write a summary report.

Step 4: Quality Assurance Committee (QAC) 

The QAC will review the coach’s report and then decide if you have successfully met your Quality requirements through the QI Enhanced program or additionally require a QA Peer and Practice assessment.
