QI for Individuals and Groups Subset

Thank you for your interest in CPSO’s Quality Improvement (QI) for Individuals Program. Please see below for an overview of the program components, which are made available for participants through the CPSO Portal.

Step 1: QI Activities Selection [View]

You will need to independently complete three activities from the list of included QI activities; the choice of activities is entirely up to you. You are welcome to complete additional activities to obtain CPD credits, to provide feedback on the menu of options within the program, and to inspire further QI ideas for your practice.

Within this step, you are also asked to provide a brief reflection on your engagement in QI over the previous 5-year period.

Step 2: Practice Improvement Plan (PIP) [View]

You will need to identify an opportunity to improve upon within your practice (i.e. identify a problem that, if solved, will result in better quality of care or services).

Upon completion and submission of your PIP, it will be reviewed by a CPSO Medical Advisor or physician QI Coach using the PIP Evaluation Report. Our CPSO Medical Advisors and QI Coaches are also available to meet with you and support you with feedback and coaching on your QI goals.

All physicians who successfully complete the QI for Individuals program will be considered to have met CPSO Quality requirements for this 5-year cycle.

Feedback

Your perspectives on the QI Program components and format are exceptionally important to us. Following the completion of Steps 1 and 2, we ask that you take a moment to provide feedback using the link available within your QI Program page in the CPSO Portal.

If you are interested in participating or would like to learn more about the QI for Individuals Program, please contact CPSO Quality staff through the Portal or by phone at 416-967-2648.

QI Groups

The QI Groups program option allows physicians to work together to develop and submit a single Practice Improvement Plan (PIP). Group members complete Step 1: QI Activities individually, then work together to develop and submit their QI Group PIP in Step 2. QI Groups also have access to the support from our CPSO Medical Advisors and QI Coaches. If you wish to form a QI group or learn more about this program option, please contact CPSO Quality staff using the information above.
