Certificate of Professional Conduct
A Certificate of Professional Conduct verifies a physician’s registration and standing with the CPSO.
It’s required by Ontario hospitals and other health facilities reviewing your application for hospital privileges or other employment. It’s also required if a doctor applies for a medical licence in another jurisdiction.
To obtain a Certificate of Professional Conduct, you must login to the Member Portal. You can download and submit the form in the “Membership Services” section of the portal.
We process the request in approximately 15 business days, but some requests may take longer. We send the official copy of the Certificate of Professional Conduct directly to the requesting organization.
Note: the certificate may include any other information about you the Registrar/CEO is aware of and considers relevant to the receiving hospital, medical school, regulatory authority or other organization. Examples of such information include:
- Reference to ongoing complaint investigations;
- Active undertakings by the member;
- Investigation outcomes;
- Third-party information about the member, such as reports from hospitals, other regulatory authorities, etc.; and/or
- Findings of professional negligence or malpractice, settlements, and charges or convictions for offences.
